Womens Team Entry Form

ENTRY FEE $220 + GREENS FEES $80 = TOTAL DUE $300
If you sign up with CASH or CHECK, the cost is $300 payable to Midstate Amusements. If you want to sign up and pay with a CREDIT CARD (only available if you played in a Midstate Amusements pool league!), there is an additional approximately 5% Paypal processing fee so the total is $315. If you played for another operator/vendor, you must sign up through that vendor. Any other entries for non-Midstate players will be refunded minus the $15 Paypal processing fee and you would need to sign up through your vendor before the deadline. If you don’t know if you are eligible to sign up here, please talk to the person who runs your league and does your league stats.

How Does My Team Qualify?

Your pool league must be WAMO sanctioned.
All team members must be female but with any CSR.
All of your league matches must have been played on WAMO Pool Charter Holder (operator) pool tables.
All players must be 21 years old and play at least nine (9) league matches on different dates from 8/15/25 to 2/11/26.
All 5 players must have qualified within the same Operator League System.
Be On Time!: All players must be ready to play at 11am on March 13 , which is earlier than usual!

How Does My Team Register?

Fill out this form completely (print a copy here to pay with cash or check and submit it to your WAMO Pool Charter Holder (operator/vendor who runs your pool league) https://midstateamusements.com/wp-content/uploads/2026/01/2026-WAMO-Pool-Tournament-Womens-Team-Entry-Form.pdf or sign up online here (scroll to the bottom of this page to sign up online) and pay with a credit card or Paypal if you played in a Midstate Amusements Pool League.

Deadline to sign up is 1/30/26

Entry fees must be paid in full when you sign up. Checks should be made payable to your WAMO Pool Charter Holder (operator/vendor) by 5pm on Friday, January 30th, 2026. So you played in a Midstate Amusements pool league; make checks payable to Midstate Amusements for the amount of your entries. Do not make checks out to WAMO. All entries include a $10 admin fee.
Late Entries may be accepted but are not guaranteed. A $50 late fee applies.

Verification

Your WAMO Pool Charter Holder (operator) must verify your eligibility to WAMO in a timely manner. Make sure you have entered all your matches into Compusport so your games count!
